FULFILLMENT POLICY
Avazach Psychiatry Services PLLC is committed to providing you with high quality psychiatric care in the most cost-effective manner possible. To accomplish this, we depend upon your prompt payment for the services provided. Avazach Psychiatry Services PLLC is not a mental health center and therefore receive no government subsidies.
You are responsible for full payment of services provided by Avazach Psychiatry Services PLLC, and any of its providers, either through your insurance policy or personal payment. Copays, when applicable, must be paid at the time of the appointment. If you do not pay your copay, your appointment will be rescheduled, and you may be charged a no-show fee.
Please call your insurance company before your first visit to find out if you need a prior authorization for services. A prior authorization is not a guarantee of services covered. Patients will be liable for any service done that insurance will not pay for. Payment (full or partial) of outstanding bills is required before subsequent appointment can be scheduled. There are no refunds for services provided unless there is a billing error, then refunds will be sent to the original form of purchase.
If you refuse to take care of your financial responsibilities, you will be discharged from treatment.
CANCELLATION POLICY
Please give Avazach Psychiatry Services PLLC at least a 24-hour notice if you cannot make it to your appointment. You may leave a message at 512-586-3465 or info@avazachpsychiatry.com, contact us through your appointment reminders, or send us notification through the patient portal. It allows us to get in other patients that need to be seen sooner. If you cancel less than 24 hours in advance or do not show up to your appointment, you may be billed a no-show fee.
FEES
Costs for an initial or follow-up adult consultations: For information about the cost of initial or follow-up adult consultations, please contact Avazach Psychiatry Services PLLC at info@avazachpsychiatry.com for details on the duration of each service, out-of-pocket fees, and any expected patient costs if you're using health insurance.
Fees for diagnostic screenings: Comprehensive standardized screens or assessments for depression, anxiety, mania, psychosis, PTSD, ADHD, OCD, etc. are included in the consultation fee.
If you require a comprehensive letter or report after a general assessment, there may be an additional fee.
Fees for correspondence sent to other healthcare providers: Avazach Psychiatry Services PLLC will not charge any fee for writing and sending standard brief primary care provider (PCP) letters or letters that are necessary for the correct clinical management of your situation. Examples: update letter to the PCP, referral letter to another specialist, letter for your health insurance (with your consent).
Fees for school or work accommodations/FMLA/LOA etc.: Form completion for school or work accommodations/FMLA/LOA etc. have a fee if the service is available. Contact Avazach Psychiatry Services PLLC at info@avazachpsychiatry.com to assess if these services are currently being offered and the cost associated with them.
Fees for prescriptions given at time of consultation or repeat prescriptions given outside an appointment: If clinically appropriate, ??your clinician will provide you with an electronic prescription that you can collect from your pharmacy during your consultation. Prescriptions issued during a consultation do not carry an admin fee.
If you request a prescription outside of an appointment there may be an admin fee. Contact Avazach Psychiatry Services PLLC at info@avazachpsychiatry.com for current information on the cost of medication refills outside of a scheduled doctor appointment.
Please be aware that most prescriptions typically include a 3-month supply of medication. Any repeat prescriptions within that period may not be covered by your health insurance, and you may need to pay your pharmacy directly for those refills.
Please note that your doctor will request a follow-up appointment at least every 6 months to write ongoing prescriptions. An initial new assessment will be requested if the patient hasn’t seen the professional for over a year.
Can I get my primary care provider (PCP) to prescribe my medication?
Yes, this is possible. You can discuss this with your clinician during your consultation. Please note some PCPs may not agree to take over the prescribing of certain medications. Your PCP may want your clinician to sign a Shared Care Agreement or may refuse to take over the prescribing entirely. In those cases, we can provide you with ongoing prescriptions.
HOW TO MAKE PAYMENTS
Payments are received by Avazach Psychiatry Services PLLC by a credit or debit card via our secure Stripe payment feature, which enables us to take a bank payment from your account only upon your agreement at the time of a service request. We will not take payments from your account without permission.
If you are funded by medical insurance, we require a pre-authorization or verification to secure your appointment. Contact your insurer before your consultation to check the terms of your policy, particularly the level and type of outpatient cover you have, including behavioral health coverage any reimbursement limits on individual consultation fees.
Please note that if your insurer does not cover the fees, you may be liable for payment.

Please be aware that Avazach Psychiatry Services PLLC may not be able to get back to you until after 2-3 business days and that depending on the situation, it may be necessary to book a consultation to discuss your query. Avazach Psychiatry Services PLLC ensures that all complaints are addressed at the earliest possible opportunity and are resolved within 28 days.

CONSENT
Before a provider at Avazach Psychiatry Services PLLC examines or treats you, they need your consent. Sometimes you can simply tell them whether you agree with their suggestions. However, sometimes a written record of your decision is helpful, so you’ll be asked to sign a consent form. If you later change your mind, you’re entitled to withdraw consent – even after signing.
PRIVACY AND CONFIDENTIALITY OF INFORMATION

MEDICAL RESPONSIBILITY OF PATIENT PSYCHIATRIC CARE
After your initial consultation with your clinician, they will be able to let you know ??if they feel that they have the right skillset and availability ??to take over the responsibility of your psychiatric care.
If your appointment is a second opinion consultation, your clinician will, with your consent, write a letter to the original referrer (and to your PCP if you agree), but they will not take over the medical responsibility for your psychiatric care or prescribe any medications.
In most cases, your clinician will be able to assume ongoing responsibility for your psychiatric care if you receive regular (at least six monthly) follow up reviews and provide permission for your clinician to share relevant information with your PCP about your care and treatment (if appropriate depending on the circumstances).
Please note that depending on your circumstances, your clinician may not be able to take over your psychiatric care or may decide to discontinue your care usually after the worsening of your symptomatology. This could be, for example, if you are feeling acutely unwell and your clinician does not have availability to safely take responsibility for your care. It is also possible that following your initial consultation with your clinician, they may find that you have a diagnosis that they do not have sufficient expertise in treating. If that was the case, your clinician would let you know after the initial consultation and would endeavor to suggest a suitable colleague with the right availability or expertise.
In cases where there is a potential risk to your health (for example if you are acutely unwell, or have relapsed into severe addiction or have intense suicidal thoughts), your clinician may decide that in order to be able to continue holding responsibility for your psychiatric care they would require you to provide consent for them to speak to a specific third party, to ensure your safety or to avoid misleading other people involved in your care. If this is the case, your clinician will discuss this with you, with a view to arriving at a mutually acceptable solution.
In certain emergency or extreme circumstances, your clinician may contact a third party (usually a family member or significant other) without your prior notification or consent. This is in accordance with the standards of practice for all psychiatrists licensed to practice in the USA.
This can happen:
- if there is a significant risk to your health or safety or that of others
- if there is a safeguarding concern regarding a child
- if there is a strong concern about your mental capacity
- if there is an urgent need for a Mental Health Act Assessment
Please note that in all these situations your clinician will endeavor to discuss the situation with you to discuss the most appropriate course of action prior to contacting a third party. In case there is no response on your side, or it is difficult to contact you despite several attempts, the professional could contact your PCP, therapist, or next of Kin without your verbal or written consent.
What happens if I need to be admitted to a mental health hospital?
If you wish or need to be admitted to a hospital or clinic, your clinician can refer you to a number of clinics in your area depending on your needs and personal preferences.
Avazach Psychiatry Services PLLC does not have admitting rights to a hospital, however, if admission is needed your provider at Avazach Psychiatry Services PLLC will be able to refer you to the nearest hospital for admission.
Please note that if you are admitted to a hospital or placed under the care of a home care or crisis team, your clinician at Avazach Psychiatry Services PLLC will no longer be responsible for managing your psychiatric care during that period. However, they may communicate with the psychiatrist overseeing your treatment if it is deemed appropriate for your situation.
What happens if I travel outside of the US or outside my state?
If you travel outside of the US or outside your home state within the US for an extended period of time, let your clinician at Avazach Psychiatry Services PLLC know so you can discuss your treatment plan and the level of support you will require.
Your clinician at Avazach Psychiatry Services PLLC holds medical indemnity insurance that allows them to provide psychiatric assessments and advice only in certain districts of the US only, although the patient can be traveling somewhere else at the time of the assessment.
If you move or live outside of the US, seek the help of a local psychiatrist or medical doctor who can assist in the event of an emergency and monitor your progress and prescribe medication if required. Any involvement from your clinician (such as emails or remote consultations) is for support purposes only and is not a substitute for appropriate medical care in the country you are located in.
